For all clinical enquiries patients should now use our online triage request form.
If you can’t use or need support to complete the online triage form, you can contact the surgery on 01724 842244 or walk-in and speak to one of our reception team, and we can do this for you.
To request GP input for medical concerns, please complete our online request form. Please note that a response should be received within 48 hours (working hours).
Once you’ve submitted the online form, a GP will review your submission and decide on the most appropriate next steps. This may include:
- An appointment at the surgery (on the same day, if clinically appropriate)
- Referral to a pharmacy (we can arrange this directly)
- Signposting to another appropriate service, such as a First Contact physiotherapist, mental health worker, or social prescriber
If you submit the triage form after 5pm you may not receive a response until the following working day
The form can be accessed through our websites Contact Us page or on the NHS app.
This service is not for emergency requests, if you need emergency medical assistance – please contact 111 or 999 in a medical emergency.
IMPORTANT
Photos - Please do not attached photos of intimate areas.
Interpreters - If you need an interpreter for any face to face or telephone consultations, please state this clearly in your enquiry.
Contact details - when completing any triage request, please ensure that all the contact details you provide are correct. This ensures that we are able to contact you either via SMS or phone call to discuss your submitted triage.
Chaperones - All patients are entitled to have a chaperone present for any consultation, examination or procedure where they feel one is required. If you would like to have a chaperone present for your appointment, we ask that where possible, this is outlined in your triage submission.
