Online Triage Information

 
To request an appointment at one of our surgeries please submit an online request form. This will be reviewed by a clinician within 72 hrs and they will decide the most appropriate action. This may include an appointment at the surgery, attending your local pharmacy, or one of our other services - first contact physio, mental health service, or social prescriber. If your total triage form is submitted after 5pm you may not receive a response until the following working day.
PLEASE DO NOT ATTACH PHOTOS OF INTIMATE AREAS.     IMPORTANT - IF YOU NEED AN INTERPRETER FOR ANY FACE TO FACE OR TELEPHONE CONSULTATION PLEASE STATE THIS CLEARLY IN YOUR ENQUIRY.

This service is not for urgent requests, if you need urgent medical assistance, please contact 111 or 999 in a medical emergency.
